Keep in mind you may not apply for a temporary handicap placard while you are in the process of applying for a permanent handicap placard.
How to Apply
Please follow the steps below in order to apply:
Obtain a temporary handicap form. Find the form online or visit the Police Headquarters Records Bureau.
Have your prescribing physician fill out the form
Return completed form to the Records Bureau and a check or money order in the amount of $4 made payable to NJ Motor Vehicle Commission, we do not accept cash or credit cards
